§ 37-97. Permit application.

All applicants doing work within the public way shall first apply for a public way permit by submitting the following to the city:

(1)

Completed application form;

(2)

Fee as established by city council;

(3)

Dates of the work proposed start and stop times, and any proposal to temporarily reopen any roadway for any period;

(4)

Lane rental/closure request;

(5)

Names of subcontractors working on the proposed project under the applicant's responsibility and authority;

(6)

Documentation of insurance coverage required by this title;

(7)

A list of the applicant's emergency contacts, including name of company, local contact person, mailing and e-mail address, 24-hour emergency phone number, and pager or fax number;

(8)

Erosion control plan;

(9)

Three sets of:

a.

Traffic control plan per MUTCD,

b.

Temporary traffic control schedule outlining public way closings for the duration of the project,

c.

Plans identifying construction routes.
